The Final Grade Report Form is the end-of-elective student performance evaluation that is to be filled out and signed by the elective director for the respective elective rotation. These forms will all be completed online.

There is a Final Grade Report Form available for Non-Patient contact (e.g. research, medical simulation) electives that you may use for reference purposes (access form below).

Some sites choose to use this electronic form to collect formative feedback throughout the 4-week elective rotations from residents, attendings, etc. This form cannot be submitted in lieu of the Final Grade Report Form. When using the electronic link, email addresses must be entered for the evaluator & whomever else needs to receive the evaluation. NEOMED does not process this form; compiling of the forms needs to be completed by the Elective Director and/or coordinator (access form below).

Our students are advised to have a discussion with their teaching faculty about their evaluation during each block. While they do not have access to the Final Grade Report Form link, they can provide any of their resident or attending faculty with the Subform link or form throughout the rotation. Students will be receiving a similar email with information about each of the forms and the role they play in their final grade.
